Who’s Eligible

Tina's World of Love is a daytime alternative to the nursing home and home care services. This facility is for disabled or older adults who want to continue living at home, but who require daytime care.

Eligible participants are disabled and/or older adults who:

Have been diagnosed with one or more physical impairments
Are not aggressive or abusive to others
Are in need of daily supervision and may require assistance with transfers and personal care

Participant Check List
- Physical
- TB test
- Insurance information
- Uniform Assesment Instrument (UAI)
- List of all medication
- Doctor's Name/Address/Phone number
- Copy of advance directives
- Two emergency contacts
